Kit: Alias by DDBN. I am pretty sure it has been discontinued, as I was unable to find it in her store. You can email Nicole through her blog or forum and she might sell it to you if you ask nicely! I used a CTMH sketch from page 20 of Imagine. The stitching is from a kit called Where the Heart Grows, I believe it is a Garden Girls kit.
